List of Supporting Documents Required at The time of Submission of Form:
- Affix recent Passport size, coloured photograph of the Child, Father and Mother on the form.
- Proof to validate first born child (Affidavit on Rs. 10/- Stamp Paper).
- Birth Certificate of Child issued by MCD/NDMC
- Proof of Residence – Attested Photocopy of Parent(s’) Ration Card, Voter’s Identity Card, Telephone Bill, Electricity Bill, Water Bill, Passport, Gas connection receipt, Driving License, Bank passbook’s first page having account holder’s details, Aadhar Card(Any two).
- Proof of Alumni of DAV Sreshtha Vihar i.e. Photocopy of class XII Certificate.
- Proof to validate child with special medical needs.
- Proof to validate Sibling Application (copy of last Fee Bill or I-Card or Report Card).
- Parents’ Identity Card issued from the work place (if available).
